Recently, in many institutes in Japan, thoracoscopic esophagectomy in the prone position has been employed for radical esophagectomy for patients with esophageal cancer. However, lymphadenectomy along the left recurrent laryngeal nerve has been thought to be a burdensome procedure because of the difficulty to obtain good operative field at the left upper mediastinum. We performed lymphadenectomy of this part without dividing the esophagus throughout this procedure. It allows the adequate tension of the left recurrent laryngeal nerve, resulting in avoiding nerve palsy. In fact, the incidence of recurrent nerve palsy was obviously low after the standardization of this procedure. We report our surgical procedures of left upper mediastinal lymphadenectomy to avoid left recurrent laryngeal nerve palsy.
