原発性肺癌に対する積極的な胸腔鏡下の肺葉切除術，特に縦隔リンパ節郭清に対する評価は定まっていない．しかし，Ⅰ期肺癌では90％以上の5年生存率が報告され，十分な長期予後が期待できる症例のグループがあることは確かであり，また縦隔リンパ節郭清を必要としない肺癌のグループがあることもわかってきている．今後はこの術式をどのように安全に広めていくか，また正しく評価するためにはどのようにすべきかが問題である．患者や内科医からこの手術が要請されてきており，将来はさらに複雑な術式が導入されてくることが考えられるため，呼吸器外科医は十分に備えておくことが望ましい．
Although video-assisted thoracic surgery (VATS) approach for a lobectomy with systematic mediastinal lymphnode dissection is reported to be performed with acceptable safety, it is still questionable whether VATS lobectomy is adequate as a radical cancer operation. However, many authors reported the 5-year survival rate following VATS lobectomy to be greater than 90% in clinical Stage I lung cancer patients. This demonstrates that there is a certain group of patients who can receive benefits from this procedure. Also it became clear that there are early stage lung cancer patients who do not require mediastinal nodal dissection.
