Thoracoscopic approaches and specific anatomy to the upper thoracic spine were described. Case 1 was 64 y.o. male with T3 paravertebral tumor, and case 2 was 12 y.o. female with T3 collapsed vertebral body treated by corpectomy and bone grafting. Patient's positioning on the operative table was crucial ; that is, sufficient space to establish working ports directly on the affected vertebral body as well as spreading the intercostal spaces were mandatory. Each port was aligned with L-shape arrangement. No postoperative axillary pain nor specific complications for the upper thoracic spine were observed.
