胸腔鏡下に摘出した臓側胸膜由来localized fibrous tumorの1例

Thoracoscopic Resection for a Case of Localized Fibrous Tumor of the Pleura

　胸部写真の経過から有茎性腫瘍と診断し，胸腔鏡下に摘出した臓側胸膜由来のlocal-ized fibrous tumorの1例を経験した．症例は74歳，女性．検診胸部写真で左肺に超鶏卵大の腫瘤陰影が突然出現したとして当科紹介となった．異常なしとされた過去の胸部写真で縦隔に腫瘤陰影が認められ，紹介時にはこの陰影が消失し肺野の陰影が出現していた．CT所見も含めて有茎性腫瘍と診断し，胸腔鏡下腫瘍摘出術を施行した．腫瘍は上葉の臓側胸膜から発生していた．エンドGIAを用いて切除しエンドキャッチを用いて摘出した．創は3か所（0.5cm,1.5cm，5.0cm）．病理診断の結果localized fibrous tumor of the pleuraと診断された．

We report a rare case of localized fibrous tumor of the pleura which was removed thoracoscopically. The patient was a 74-year-old female who had an abnormal shadow on a chest X-ray film. The shadow was diagnosed as a pedunculated mass because of the changes in location from mediastinum to interlobar space on chest X-ray films. The operative and macroscopic findings revealed that the tumor was well encapsulated and originated from visceral pleura of the left upper lung and projecting into the pleural cavity as a pedunculated mass. The tumor was diagnosed as a localized fibrous tumor of the pleura microscopically.
