副腎腫瘍に対する側臥位・側方アプローチによる経腹的腹腔鏡下手術

Lateral Transabdominal Laparoscopic Adrenectomy for Adrenal Tumor

　副腎腫瘍に対する内視鏡下手術の到達法には，経腹的前方，側方アプローチおよび後腹膜的側方，後方アプローチとあるがいずれも一長一短があり，施設により手法は異なる．今回われわれは良好な視野が得られ，腸管の副損傷の危険性も少ない経腹的側臥位・側方アプローチにより，3例の副腎腫瘍の手術を行った．特に最近の2例では，アルゴン・ビーム　コアギュレーター^®，ハーモニック・スカルペル^®，Ultrasonic Surgical Unit^®を使用しながら手術を行うことで手術手技はより容易となった．3例の平均手術時間は190分，平均出血量は63mlであった．本法は副腎腫瘍に対する安全で確実な推奨されるべきアプローチ法と考えられる．

Several kinds of approaches have been reported for endoscopic adrenectomy. However, there are some different technical advantages or disadvantages in each approach. In the process of transition from open technique to endoscopic technique, we selected the lateral transabdominal laparoscopic adrenectomy because of wide vision and less possibility of bowel injury. Our three experiences performed by this approach was easier to operate resulting in good course. In addition, we operated more safely by using Argon Beam Coagulator, Harmonic Scalpel and Ultrasonic Surgical Unit.
