気腫性肺嚢胞に対する胸腔鏡手術症例の検討

Thoracoscopic Treatment of Emphysematous Bullae: Its Technical Aspects

　巨大肺嚢胞を含む気腫性肺嚢胞の胸腔鏡手術症例をretrospectiveに検討した．1993年3月から1996年12月までに南一条病院で手術を行った気腫性肺？胞は24例28側で，全例で胸腔鏡下に嚢胞の切除，縫縮を行った．1例で技術的な理由から開胸手術に移行した以外は術中の問題はなかった．2例3側で8日間以上の気漏の遷延がみられ，2例2側で閉鎖術を施行した以外は術後の合併症はなかった．これらはいずれも初期の症例であった．気腫性肺嚢胞の胸腔鏡手術は技術的に難度は高いものの，慎重に行えば基本的には安全な術式であり，患者へのメリットが大きい．胸腔鏡手術の手技上の問題を中心に，症例の呈示とともに報告した．

We retrospectively analyzed 24 consecutive patients treated by thoracoscopic procedure of em-physematous bulla since March 1993 through December 1996. The indications for the procedure were enlargement of bullae (4 bilateral) in 24 cases, complicated by single lung pneumothorax in 7 cases, and complicated with single lung infection in 2 cases. One patient was converted to an open procedure due to technical reasons. Postoperative complications included air leaks lasting for longer than 8 days in 2 patients, one with air leak in both lungs.
