原発性肺癌に対する胸腔鏡下手術—肺葉切除とリンパ節郭清

Video-assisted thoracoscopic lobectomy for primary lung cancer:Technique and focusing on lymph node dissection.

　VATS（video-assisted thoracoscopic surgery）は良性疾患の縮小手術から原発性肺癌に対する拡大術式として検討されている．しかし，これらが今後，悪性疾患の術式に対する1つとして受け入れられるためには，標準開胸と同様の安全性と根治性が得られることが必要である．原発性肺癌に対するVATS Iobectomyについて，これらの点に留意し，われわれが行っている原発性肺癌に対するVATSを用いた肺葉切除術，リンパ節郭清について，その基本手技と各肺葉別手技について解説を行った．

Video-assisted thoracoscopic surgery (VATS) have been applied to benign diseases. Recently, with the development of video technology, VATS is being applied to treat patients with primary lung cancer. However, in order to establish a validity with the operative procedure for treating primary lung cancer comparing to those of lobectomy under standard tho-racotomy, radical efficacy as well as safety of VATS lobectomy is a crucial point. Focusing on lymph node dissection, we showed basic technique and each lobectomy undergone by VATS.
