進行肺癌に対する胸腔鏡下肺葉切除・リンパ節郭清術

Video-assisted Thoracic Surgery (VATS) Lobectomy and Lymphadenectomy for Advanced Lung Cancer

　胸腔鏡下手術手技の進歩に伴い，肺癌も胸腔鏡手術の適応となった，われわれは，原則として臨床病期I期肺癌を胸腔鏡下肺葉切除・リンパ節郭清術の適応としてきたが，進行癌のなかにも本術式の適応となるものがある．本稿では，進行肺癌に対する胸腔鏡下肺葉切除・リンパ節郭清術に関するわれわれの考えを述べた．

With the development of techniques in thoracoscopic surgery, lung cancer has become a feasible indication for VATS lobectomy combined with lymphadenectomy. Although stage I lung cancer is one of the most adequate indications for this surgical procedure, some advanced lung cancer cases are also an indication for this tech-nique. Herein we describe our concept regarding VATS lobectomy and lymphadenectomy toward advanced lung cancer.
