内視鏡下鼠径部ヘルニア修復術—外鼠径，大腿および閉鎖孔ヘルニアを合併した1例

Endoscopic herniorrhaphy:A case report of indirect inguinal hernia combined with femoral and obturator hernia.

　鼠径部ヘルニアの治療に，従来のanterior approachに加え内視鏡下手術が導入され始めている．内視鏡下手術の長所は，術後の疼痛が軽く早期社会復帰が可能なことに加え，ヘルニアの正確な診断と，同一術野で多発ヘルニアの治療が同時に可能なことである．われわれは，47例の鼠径部ヘルニアの治療を内視鏡下で行い，外鼠径，大腿および閉鎖孔ヘルニアを合併した1例を経験した．大腿，閉鎖孔ヘルニアは不顕性で，anterior approachでは存在が見落とされると思われたが，内視鏡下手術では診断が容易で，術後第2病日目に退院し，1995年8月（術後9か月）現在再発は認めていない．このような症例は，これまでに報告されていない．内視鏡下手術の有用性を示す1例と思われるので報告する．

Endoscopic surgery has been introduced for the treat-ment of inguinal hernia in addition to conventional anterior approach method. Endoscopic surgery has advantages such as : less postoperative pain, sooner return to daily life and correct diagnosis and treatment for multiple hernias in the same field of operation. We have experienced endoscopic herniorrhaphies in 43 patients and one of them had indirect inguinal hernia combined with femoral and obturator hernias. The femoral and obturator hernias were not clinically obvi-ous and might be left unnoticed with conventional anterior approach.
