　鼠径部の膨隆として出現する外膀胱上窩ヘルニアは，術前診断が難しい．症例は49歳，男性．主訴は立位で出現する右鼠径部の無痛性膨隆．腰椎麻酔下で腹膜外腔を剥離し，炭酸ガスを注入して拡張した．内視鏡で観察すると，ヘルニア門は腹直筋恥骨付着部の右側・Cooper靱帯頭側に位置し，大きさは1×1cmであった．ポリプロピレンメッシュでヘルニア門を含む鼠径床部を被覆し，ヘルニアステイプルで固定した．術後第3病日に退院した。外膀胱上窩ヘルニアに対するtotally extraperitoneal preperitoneal-laparoscopic herniorrhaphyの報告は本例が第1例目と思われるので報告する．

The supravesical hernia, which usually manifests as a painless tumor in the inguinal region, is difficult to diagnose preoperatively. We have experienced a case of supravesical hernia in a 49 year-old male. His chief complaint was painless right inguinal tumor appearing on standing position. Under spinal anesthesia, on tenta-tive diagnosis of inguinal hernia, we dissected the pre-peritoneal space and dilated it with carbon dioxide gas (total extraperitoneal approach) .
