For medical professionals who have been involved in blood purification therapy in the intensive-care unit, the question of whether or not continuous renal replacement therapy (CRRT) improves the prognosis in critically ill patients with acute kidney injury has been a very important issue. Various efforts have been made, including the selection of hemofilters and modalities and the start timing and flow rate of purification liquid, but the “best” combination has never been clarified. Nevertheless, hemofilters with new functions have been developed and put on the market one after another. This is the history of progress in Critical Care Nephrology. CRRT using an AN69ST membrane can now be used for cases of severe sepsis and septic shock, even without acute kidney injury, and we are actively using the AN69ST membrane in our practice. Selecting a hemofilter according to the pathology and performing appropriate treatment are highly likely to improve the prognosis
