13．ヘモフィルターについてのPros & Cons—Pro：予後を改善する

Continuous renal replacement therapy helps improve the prognosis

ICUでの血液浄化療法にかかわってきた医療従事者にとって，急性腎障害（AKI）を有する重症患者における持続的腎代替療法（CRRT）が予後を改善するかという課題は非常に重要なものであった。ヘモフィルターやモダリティの選択，開始時期や浄化液流量の調整などさまざまな取り組みがなされてきたが，「最良」といわれる組み合わせが明確になったことはない。それでも，新しい機能を有するヘモフィルターが次々に開発・上市されてきた。これはcritical care nephrologyの進歩の歴史でもある。AN69ST膜を用いたCRRTがAKIを合併していなくても重症敗血症・敗血症性ショックに用いることができるようになり，積極的に使用している。病態に応じてヘモフィルターを選択し治療を行うことが予後を改善する可能性は，十分に高いと考えられる。

For medical professionals who have been involved in blood purification therapy in the intensive-care unit, the question of whether or not continuous renal replacement therapy （CRRT） improves the prognosis in critically ill patients with acute kidney injury has been a very important issue. Various efforts have been made, including the selection of hemofilters and modalities and the start timing and flow rate of purification liquid, but the “best” combination has never been clarified. Nevertheless, hemofilters with new functions have been developed and put on the market one after another. This is the history of progress in Critical Care Nephrology. CRRT using an AN69ST membrane can now be used for cases of severe sepsis and septic shock, even without acute kidney injury, and we are actively using the AN69ST membrane in our practice. Selecting a hemofilter according to the pathology and performing appropriate treatment are highly likely to improve the prognosis
