腎代替療法（RRT）は重症の急性腎障害（AKI）に対して行われる介入である。2016年から立て続けにRRT開始のタイミングについて臨床試験や二次解析の結果が発表され，AKIに対するRRTの立ち位置が明らかになりつつある。重症なAKIと診断がついても，直ちにRRTを行わなくとも自然に腎機能が回復することが多く，呼吸不全や電解質異常がひっ迫するまではRRTを控える流れに変わってきている。本稿では，RRT開始のタイミング，モダリティの選択，離脱について，これまでの研究からわかったこと，いまだにわかっていないことを紹介する。

Renal replacement therapy （RRT） is an intervention performed for severe acute kidney injury （AKI）. Randomized clinical trials and accompanying data analysis on the optimal timing for initiation of RRT have been published since 2016, and the position for RRT in the management of AKI is becoming clearer. Even if a patient is diagnosed with severe AKI, the trend has changed to delaying RRT until strongly indicated by clinical signs and symptoms as in many cases, kidney function can naturally recover without immediate RRT. In this review, we will provide an overview of the available evidence to date regarding the timing of RRT initiation, the choice of RRT modality, and the discontinuation of RRT as well as what is still unknown.
