4．clinical trial group（CTG）とは—学術的な生産性や仲間意識だけでなく，守秘義務もCTGの維持と成長に必要である

重症症例，外傷症例の予後を改善するためには，臨床試験と観察研究は不可欠である。clinical trial group（CTG）（多施設共同臨床研究グループ）は共同研究を促進し，最新の知見を共有し，将来の研究アイデアについて議論を重ねる目的で，それぞれの国または地域（北米，ヨーロッパ，オセアニアなど）で結成されてきた。

　基本的にCTGはボランティア活動で支えられているが，個々のCTGの構造，ミーティングスケジュール，そしてアウトカムはさまざまである。しかし，学術的な生産性（論文発表など），メンバー同士の仲間感と友好関係，そして議論した内容の守秘義務と盗用の禁止はCTGが維持，成長していくうえで，不可欠な要素である。

　CTGの将来的な方向性としては，platform trialのような新しい臨床試験のデザインの導入，メンバーの所属するICUで共同参加，共同運用するデータベースの構築，そして複数のCTGが協力する共同研究が考えられる。

Rigorous clinical trials and observational studies are essential to improve clinical care for critically ill or injured infants and children. The Clinical Trial Group （CTG） was formed based on geographic regions to facilitate collaborative studies and exchange updated knowledge and findings and brainstorm future research ideas.

　While each CTG organization is maintained on a voluntarily basis, the structure, meeting schedule, and outcomes are different. Academic productivity, collegiality and friendship, and confidentiality agreements are essential for CTGs to sustain and grow their membership and increase activities. Future directions of the CTG include an implementation of new trial designs such as platform trials, generation of a big data collaborative across the CTG member ICUs, and collaboration among large CTGs.
