Malnutrition is common in patients with chronic obstructive pulmonary disease (COPD) and is associated with a poorer prognosis and impaired long-term survival. In stable malnourished patients with COPD, nutritional supplementation significantly increases weight and respiratory muscle strength and improves overall health-related quality of life. In acute care settings, however, the evidence for benefits of nutritional therapy in patients with COPD is lacking. Caution is needed to avoid the complications of nutritional treatment. This therapy may increase oxygen consumption and carbon dioxide production. In this article we discuss “good” or “not-so-bad” nutritional treatment in patients with COPD and hypercapnia.
