7．感染性膵壊死—侵襲的介入から，低侵襲のstep-up approachという考え方へ

壊死性膵炎は急性膵炎の重篤な局所合併症であるが，保存的に治療でき，死亡率も10％未満である。ただし感染を併発すると，以前は開腹ネクロセクトミーが行われたが，その死亡率は30％前後と予後不良であった。しかし，近年，感染性膵壊死に対する治療戦略に大きな変革があった。保存的加療を選択することもまれに可能とされ，介入する場合には，step-up approach法がとられるようになった。すなわち，まず侵襲の少ない，内視鏡的あるいは経皮的ドレナージを行い，改善がみられない場合のみ感染性壊死組織のさらなる確実な除去を行う。その際も，より侵襲の少ない内視鏡下または腹腔鏡下，後腹膜経路などのネクロセクトミーが選択されるようになった。これらの適応，方法，介入時期，長期予後等に関しては，今後，より質の高い研究での評価が必要である。

Pancreatic necrosis occurs as a local complication of acute pancreatitis, and if infection complicates pancreatic necrosis, the mortality increases to 30％ with necrosectomy. In the last three decades, there has been a revolution in the treatment of infected pancreatic necrosis. Although non-operative treatment can be selected infrequently, a step-up approach is usually selected. Less invasive endoscopic or percutaneous drainage is chosen first, reserving operative intervention for patients for whom the previous procedure did not solve the problem definitively. Indication, methods, timing of intervention and long-term outcomes should be evaluated in high quality studies in the near future.
