Pancreatic necrosis occurs as a local complication of acute pancreatitis, and if infection complicates pancreatic necrosis, the mortality increases to 30% with necrosectomy. In the last three decades, there has been a revolution in the treatment of infected pancreatic necrosis. Although non-operative treatment can be selected infrequently, a step-up approach is usually selected. Less invasive endoscopic or percutaneous drainage is chosen first, reserving operative intervention for patients for whom the previous procedure did not solve the problem definitively. Indication, methods, timing of intervention and long-term outcomes should be evaluated in high quality studies in the near future.
