While treatments for acute cerebral infarction have developed dramatically, no remarkable progress in the treatments of acute intracerebral hemorrhage (ICH) has been noted. There is no clear evidence showing the efficacy of surgical treatment for ICH. This article discusses the current status of the surgical treatment of ICH, using the latest major guidelines and recent key studies such as the STICH trials. Clinical experience and minimally invasive treatment of patients with ICH are also described. The negative results of the STICH trials do not eliminate the utility of early surgical treatment of supra-tentorial hemorrhage. Several meta-analyses suggest that the surgical treatment of ICH is effective for certain patient subsets. Further studies are needed to make definitive conclusions. Reports of minimally invasive surgery for ICH are recently increasing, with promising results. The need for minimally invasive surgery will further increase in the future.
