Aneurysmal subarachnoid hemorrhage is a neurological syndrome with complex systemic reactions and complications. Since systemic complications can worsen ongoing intracranial pathology, neurocritical care including systemic organ support is essential. Maintaining euvolemia is recommended, avoiding hypovolemia or preventive hypervolemia. Since nimodipine is not available in Japan, continuous nicardipine infusion and intravenous fasudil are alternative drugs of choice for the prevention of delayed cerebral ischemia (DCI). When DCI is diagnosed, induction of hypertension and mild hypervolemia is recommended. The aim of this article is to summarize neurocritical care for patients with subarachnoid hemorrhage due to ruptured aneurysms, especially for patients in poor neurological condition due to early brain injury and DCI.
