精神障害者の社会適応モデル—生活の満足と自己価値に関するパス解析［第2部］

Communityy Adjustment of Chronic Schizophrenics in Japan: Effects on Life Satisfaction and Self-Esteem

　在宅で外来通院を継続する精神分裂病者120名を対象に，社会適応モデルをテストした。このモデノしは，疾病の2側面，すなわち病状の重篤さと病歴としての慢性度が，社会生活への適応に負の影響を及ぼし，また，生活への適応の如何が，生活の満足や自己価値という1観的側面に影響するという，社会適応のありかたを概念化したものである。

　結果は，仮説としてのモデルの一部を検証し，一部は、立証されなかった。すなわち，疾病要因の生活適応への影響については，慢性度や病状は，口常生活適応，就労適応，家族関係適応に負の影響を及ぼしていた。しかし，友人関係適応については立証されなかった。その理由として，友人関係適応の測定上の問題を指摘した。

　生活の満足や自己価値に関しては，仮説をくつがえして，自立要因である日常生活適応および就労適応はなんの影響ももたらしていなかった。もっぱら，対人関係要因のもたらす影響が顕著にあらわれた。

　The purpose of this study was to test two community adlustment models of the chronically mentally ill developed by this author. The study was to determine the effects of daily living. work, family, and social adjustment on life satisfaction and selfesteem as well as to determine the effects of chronicity and symptomatology on the four adjustments.

　Twenty schizophrenic inpatients and 120 schizophrenic outpatients from a psychiatric hospital in Tokyo participated in the study: the former group was used for a pilot study of the instruments and the latter for the model testing.
