経鼻的整復術により改善を認めた眼窩内側壁単独骨折の1症例

A Casc of Old and Isolated Blowout Fracture of the Medial Orbital Wall Improved by Endonasal Release

I．緒言

　blowout fractureというと一般に眼窩底部の骨折を指す。Smithら^1）はその発生機序について「眼窩を密閉するように作用する鈍的外力により急激に眼窩内圧が上昇し，眼窩下壁を上顎洞方向へ吹き抜けるように骨折が起こる」と報告している。眼窩の内側壁骨折が底部骨折にかなり高率に合併することは以前より報告されていたが，最近ではCT scanなどの画像診断技術の向上も手伝い，内側壁単独骨折の報告が増えつつある。われわれは複視を訴えた眼窩内側壁単独骨折の1症例に対し経鼻的手術を施行し良好な結果を得たので，手術適応の決定および手術法などを含め若干の考察を加え報告する。

　In a 18-year-old male with an old and isolated blowout fracture of the medial orbital wall, diplopia gradually improved and disappeared one year later, although the trapped muscle had degenerated before operation. Endonasal approach under a microscope was sufficient to release the trapped medial rectus muscle from the fractured bony wall.
