眼痛を主訴とした陳旧性眼窩内側壁骨折の1例

A Case of Old Blow-Out Fracture of the Medial Orbital Wall Presenting with Eye Pain

　はじめに

　眼窩内側壁骨折は保存的に経過をみていても複視などが改善する場合が多く，自然治癒が90％とも報告されている^1）。一方で，眼窩内側壁骨折を放置したことによる晩発性の副鼻腔への障害も報告されており^1〜3），本疾患に対する手術的療法の是非に関しては論議の一致をみていない。

　今回われわれは，受傷約20年後に眼痛および視野欠損が出現した陳旧性眼窩内側壁骨折の1症例を経験したので報告する。

A 30-year-old male presented with eye pain due toold blow-out fracture of the medial orbital wall. He reported a history of a blow to the eye region and recov-ered double vision at the age of 12. He was diagnosed as having rhinogenous optic neuritis with a slight disorder of visual acuity and a slight defect of visual field. These disorders were recovered completely after surgical inter-vention. Computer-aided endoscopic sinus surgery was very useful for ethmoidectomy in this case.
