副鼻腔根治術時に見つかった陳旧性眼窩内側壁吹き抜け骨折の1症例

A Case of Old Blow-out Fracture of the Medial Orbital Wall Discovered at Sinectomy

　はじめに

　眼窩吹き抜け骨折（blow out fracture）は，眼部に加わった鈍的外力により生じた眼窩壁の骨折で，眼球内容が副鼻腔内に突出することにより複視・眼球運動障害・眼球陥凹などの特異な臨床症状を呈する疾患であり，診断は比較的容易であるが，その治療方針，特に手術適応，時期などについてさまざまな意見があり，統一された見解が示されていないのが現状である。

　今回我々は慢性副鼻腔炎患者で，術前の問診では眼球打撲および複視の既往は聴取できず，手術直前の断層撮影および副鼻腔根治術時に見つかった陳旧性の眼窩内側壁吹き抜け骨折を経験したので，問診やX線像読影の重要性と共に吹き抜け骨折の治療方針についても若干の考察を加え報告する。

An old blow-out fracture of the medial orbital wall was found during sinectomy in a 56-year-old female who had suffered from discomfort of cheek and had diagnosed as chronic sinusitis. Although there was no past history regarding to her eyes, on the operation day we found the abnormal pro-truding shadow out of the right orbital lesion by x-ray tomography. During sinectomy we found a projective object in the right ethmoidal sinus. There found a history of hitting her eye and recovery of double vision in six months 15 years ago.
