白血病化に先行して上顎洞に発生したgranulocystic sarcoma（myeloblastoma）の1例

Preleukemic Granulocystic Sarcoma of the Maxillary Sinus: Report of a case

I．はじめに

　腫瘤形成性白血病は，一般に白血病の経過中，または白血病化と同時に発見されることがほとんどであり，白血病化に先行して発見されることはきわめてまれである。ここでいう白血病化とは，末梢血液所見や骨髄穿刺所見において腫瘍の性格を帯びた白血球ならびに幼若細胞が認められるようになることをいう。腫瘤形成性白血病のうち，骨髄性幼若細胞より成る骨髄芽球腫（myeloblastoma）では，急性骨髄性白血病への急性転化などと密接に関連し，予後不良の前兆と理解されている。

　今回われわれは末梢血や骨髄への白血病細胞の浸潤に先行して，まず副睾丸に原発したあと，上顎洞および頸部にmyeloblastomaが限局して発生した腫瘤形成性白血病のまれな1例を経験したので，若干の文献学的考察を加えて報告する。

　A 41-year-old male with granulocystic sarcoma in the right maxillary sinus was reported. A granulocystic sarcoma is a tumor composed of immature cells of myeloid series. It usually occurs as a secondary manifestation of acute myeloblastic leukemia. However, in this case, granulocystic sarcoma was found at least 18 months prior to the development of blood and bone marrow involvement of acute leukemia.

　It can be emphasized that granulocystic sarcoma should be taken into consideration in its diagnosis and treatment.
