上顎洞に発生したspindle cell sarcomaを手術治療した1例

A case of spindle cell sarcoma in the maxillary sinus treated by surgery

はじめに

　頭頸部領域で肉腫は非常な稀な疾患である。上顎悪性腫瘍のうち，非上皮性のものは10％程度であり，頭頸部が原発である肉腫は成人の肉腫のなかで約5〜15％である^1）。肉腫は治療プロトコールが確立しておらず，横紋筋肉腫などの一部の組織型を除き，化学療法・放射線療法は感受性が悪いため，外科的切除が治療の基本となる。しかし，頭頸部領域では広範な一塊切除は困難なことが多く，頭蓋底浸潤により切除が不可能である場合も少なくないため，慎重に治療方針を検討する必要がある。今回われわれは，上顎洞に発生したspindle cell sarcomaの症例を経験したので報告する。

　Head and neck sarcomas are rare tumors, accounting for only 1％ of all head and neck malignancies and 5％ of all sarcomas. Surgical resection is the basic treatment for sarcoma because a treatment protocol has not been established and chemotherapy and radiation therapy are ineffective except for rhabdomyosarcoma. However, in the head and neck region, extensive mass resection is often difficult, and surgery may not be possible because of skull base invasion. Thus, treatments for sarcoma should be carefully considered. Here, we report a case of spindle cell sarcoma in the maxillary sinus treated by surgery.
