Sudden Deafness Treated by Stellate Ganglion Blocks: A Review of 35 Cases

I．はじめに

　ペインクリニックで扱う疾患の中には，本来他科が扱う疾患も多く含まれている。突発性難聴もそうした疾患の一つで，従来より星状神経節ブロックが有効と考えられており，その意味ではペインクリニックの対象と考えられる疾患である。

　突発性難聴は，急激に発症する原因不明の感音性難聴で，vascular説，viral説等の種々の成因が考えられているが，最終病態としては内耳の循環障害に基づくhypoxiaと，それに結果する代謝障害との観点が有力である。現在行われている治療もこうした内耳の循環改善に重点が置かれ，血管拡張剤，循環促進剤，ビタミン剤等の薬物療法と高圧酸素療法等が主となっている。

　The cause of sudden deafness is thought to be vascular or viral in origin, resulting in reduction of blood supply to the cochlea. Stellate ganglion block is thought to be a useful therapy, as it may increase the blood supply to the cochlea.

　Thirty-five cases were treated by stellate ganglion block in the last 3.5 years, and 62.9% of the cases showed more than 10-dB average pure tone gain. Significantly, 92.9% of the improved cases were treated within the first two weeks following the onset of symptoms.

　The delay of time from the onset of symptoms to start of the stellate ganglion block therapy appears to be important.
