星状神経節ブロック施行後の咽後間隙血腫

Retropharyngeal Hematoma following Stellate Ganglion Block

　はじめに

　星状神経節ブロック（以下SGB）は適応範囲が広く，ペインクリニックで頻用されているが，耳鼻咽喉科領域でも突発性難聴などの治療に応用されている。一方SGB施行に際しての合併症は，星状神経節の位置的な関係から種々のものが挙げられるが，その多くは一過性のものである。しかし，今回SGB施行後に咽後間隙に血腫を生じ，緊急の血腫除去術を要した症例を経験したのでここに報告する。

A 43-year-old female developed a cervical swell-ing and dyspnea after she had received stellate ganglion block. A huge swelling of posterior wall of the pharynx was proved by laryngoscopic examina-tion and cervical x-ray. An intratracheal tube was inserted in emergency because air way obstruction was progressive. CT revealed a retropharyngeal hematoma. An emergency operation for the removal of hematoma and hemostatis was perfor-med. After the operation, she recovered uneventful-ly. In case of such a retropharyngeal hematoma, open procedure should be immediately performed to avoid mediastinal infection and other complica-tions.
