突発性難聴の治療についての検討—特に星状神経節ブロックとステロイドおよびプロスタグランディンの併用について

The Evaluation of the Therapy for Sudden Deafness:Especially with Stellate Ganglion Block and Steroid and/or Prostaglandin

　はじめに

　突発性難聴は急激に発症する感音難聴で，血管の攣縮，ウイルス感染，炎症，アレルギー反応などによる内耳循環障害が原因と考えられている^1,2）。急性期には，ステロイド，循環改善剤，ビタミンB₁₂などの経口投与または点滴静注を中心とした治療を行うことが多い。また，点滴治療以外の治療方法として星状神経節ブロック（以下，SGBと略）や高気圧酸素療法なども行われている。今回われわれは，突発性難聴の治療，特にSGBの有用性について検討を加えたので報告する。

70 patients suffering from sudden deafness were treated in Keiyu hospital between January 1995 and December 1997. They were divided into four groups according to the method of treatment. The follow-ings are the conclusion of this study.

(1) The results of drug therapy were similar to the others reported. (2) Patients with severe hearing impairment were treated with the combination of stellate gan-glion block (SGB) and drug therapy. Their average of the hearing level significantly improved. (3) In the cases which were treated with SGB after drug therapy, the effective rate increased to 58.8% from 17.6%. And the successful cases were sensitive to both drug therapy and SGB.
