気管支ファイバースコープによる術後性肺合併症の治療経験

Bronchofiberscopic Treatment of Postoperative Pulmonary Complications

　I．はじめに

　全麻手術後の術後性肺合併症は，麻酔法の進歩，抗生剤の開発，呼吸管理技術の向上などによって予防や治療が可能となってきたが，しかし現在でも未だ時折偶発し，老齢者や全身状態の不良の者の中には不幸な転帰をとる症例もあり，重要な問題である。従来は，このような症例に対し，喀痰の自己喀出訓練，体位による排出，振動による排出促進，盲目的吸引，などが薬物治療と合わせて行われて来た。

　最近，筆者らはこのような術後性肺合併症に対して，気管支ファイバースコープによる観察下に，気管・気管支内の分泌物の吸引や気管・気管支内の洗浄を治療として行い，好結果を得ることができたので，この経験を報告し，ご参考に供する次第である。

　Bronchofiberscopic aspiration and irrigation of postoperative pulmonary complications showed a good result in our patients.

　"Postoperative pneumonia" is not pneumonia but atelectasis due to accumulation of intrabronchial secretion and exudation. The best treatment of such complication is endoscopic aspiration, and bronchofiberscopic aspiration and irrigation were performed more easily and safely than aspiration by the use of conventional rigid bronchoscope.

　Therapeutic use of bronchofiberscope was discussed in this paper.
