巨大な鼻咽腔血管線維腫の2例と本邦報告例についての文献的考察

AN ENORMOUS NASAL ANGIOFIBROMA

　Ⅰ．緒言

　鼻咽腔線維腫は比較的稀な腫瘍であり，病理組織学的には良性腫瘍であるが，臨床的には治療困難で再発しやすいことから，悪性腫瘍として取り扱われている^{4）7）11）20）}。鼻咽腔悪性腫瘍については，臨床的統計的研究業績は現在までにも，すでに数多く報告されているが，鼻咽腔線維腫についての統計的観察は比較的少ない^17）。われわれは最近，29歳男子ならびに15歳男子に発生した鼻咽腔線維腫症例を治療する機会を得たので，まずこれについて報告するとともに，明治36年から昭和46年に至る69年間の本邦報告例271例に，われわれの治験例を加えた273例について，種々の項目に対する統計的な観察を行ない，さらに文献的考察を加えて諸賢の参考に供したいと思う。

　Two cases of nasal angiofibroma are reported.

　The first case: A man, aged 29, complained of repeated epistaxis and a severe impairement of nasal breathing. Examination showed a large angiofibroma filling the nasal cavity. The tumor was removed surgically through palatine approach by first preparing the patient with ligation of the external carotis artery and preliminary tracheotomy. The anesthesia was administered through the tracheotomy tube.

　The second case: A boy, aged 15, was found to be affected with angiofibroma of the nasal cavity, similar to the first case complaining of nasal closure, persistent epistaxis and exophthalmia. The tumor was removed surgically through palatine approach with preliminary ligation of the external carotid artery and tracheotomy.

　Literatures on similar subject reported in this country are thoroughly reviewed.
