胃・結腸・喉頭三重癌の1例

A CASE OF TRIPPLE CANCER ASSOCIATED WITH STOMACH, DESCENDING COLON AND LARYNX

　I．緒言

　同一個体に複数の原発性悪性腫瘍が認められることは，一般的に重複癌として取り扱われ，単一癌の発生する場合に比し稀なものとされている。このような原発性多発性悪性腫瘍の発生母地に関し，頭頸部領域における重複癌の報告例は少ないが，三重癌としての症例はさらに稀なものと思われる、横山ら^1）によれば，本邦における喉頭癌に伴う重復癌は1978年までに105例報告されているが，さらに喉頭癌と重複する三重癌は本邦文献を渉猟し得た範囲では，現在までに8例報告されているに過ぎず^2〜9）極めて稀なものと思われる。

　今回筆者らは胃癌手術後6年を経て結腸癌が発見され，さらにその8カ月後に喉頭癌がみつかった三重癌の1例を経験したので，本例の臨床経過とあわせて文献的考察を述べ，三重癌に関する統計的観察も行いたい。

　A case of tripple cancer in a 77-year-old man was reported. At the age of 69, he underwent surgery for gastric tumor, and the pathohistological finding showed medullary adenocarcinoma. Six years later, the patient was operated on with the diagnosis of tumor of the descending colon which was histologically confirmed as adenocarcinoma. Furthermore eight months later, he was found having the third tumor (squamous cell carcinoma of the larynx). The patient was treated by microlaryngosurgery and postoperative radiotherapy. Eight cases of tripple cancer associated with laryngeal cancer had been reported from 1964 to 1978.
