口腔内アフタと血便を伴った1症例—抗生剤のside effectか

REPORT OF A CASE OF APHTHOUS STOMATITIS WITH BLOODY STOOL: SUSPICIOUS SIDE EFFECTS OF ANTIBIOTIC

　I．はじめに

　最近の抗生剤の開発およびその効力には注目すべきものがあるが，一方では当初予想だにされていなかった副作用が報告されつつある。消化管に対する副作用としては，linconiycin，clindamycinなどによる偽膜性大腸炎^1,2）が代表であるが，筆者らは最近，急性扁桃炎に対して抗生剤使用後，原因不明の口内炎と血便を呈した症例を経験したので，報告する。

　A male patient, aged 31, was referred to our hospital by a practitioner with a suspicion of Behçet disease after the patient was treated with antibiotics (ciclacillin, amoxicillin, minocycline, cefazolin and lincomycin) for acute tonsillitis. On admission, intestinal Behçet disease, leukemia, malignant lymphoma or drugs reaction should be ruled out because the patient presented oral aphthous ulcers, skin eruptions resembling folliculitis, and bloody diarrhea. Finally antibiotic-associated hemorrhagic colitis was diagnosed and the patient was treated.
