 Forty eight ears of ossicular reconstruction with intact stapes in tympanoplasty are presented and their hearing results are analyzed with reference to the type of reconstruction employed. In cases of dislocation of the incudostapedial joint or in the loss of the lenticular process of the incus, the lenticular process or the long process of the incus was brought into contact with the head of the stapes. In the loss of a part of the long process of the incus, a cartilage chip was interposed between the shortened long process of the incus and stapes head. With the loss of the long process or body of the incus or both, the body of the incus or a cartilage is used as a free graft to connect the stapes to the handle of the malleus. In a case with the loss of malleus and incus, cartilagenous strut is placed on the top of the stapes.

 The percentage of hearing gains that retained 15 dB by closing the air-bone gap in the four types of operations varied from 14 to 57%, the average being38%; while that of a socially adequate hearing level varies from 46 to 73%, with the average being 55%.

 The highest percentage of successful hearing gain was found when free graft was made with the body of the incus or cartilage ; the lowest was with the type using cartilage strut. No cases are found with increased hearing loss of sensorineural type and a postoperative air-bone gap greater than 40 dB.
