鼓室成形術III型変法の成績—キヌタ骨による連鎖再建

Functional Results of Type III Tympanoplasty with Ossicular Reconstruction

　I．緒言

　鼓室成形術III型変法とは，健全なアブミ骨，あるいは少なくとも上部構造の高度な破壊がないアブミ骨を基盤として，その上に補填物を置き，連鎖の再建が図られた場合のことである。現在普通行われている外耳道後壁を保存する鼓室成形術にあっては，十分な鼓室腔を確保し，なるべく自然な位置に鼓膜を保存，あるいは再形成するためにも，この方法は理にかなった方法である。

　補填材料については，最近異物反応が少なく，組織親和性のあるテフロン系物質やアルミナセラミックスを素材としたPORPが炎症のない耳に応用されて脚光を浴びてきているが，一般にはもっぱら自家耳小骨や同種耳小骨，それに自家および同種軟骨が使われている。

　当科における昭和50年以前のIII型変法の成績は，昭和48年，49年の2年間に手術を施行した48耳において，4分法による気導聴力30dB以内55％，気導聴力改善15dB以上54％，A-B gap 15dB以内38％，同20dB以内54％であった^1）。

　この際，ツチ骨とアブミ骨を軟骨や骨あるいはいったん取り出したキヌタ骨で連結する方法が再建法としては優れていた。したがってそれ以降はこのいわば代用キヌタ骨法を多用してきた。本論文においては，昭和50年から6年間の成績を報告するとともに，この代用キヌタ骨法を再評価し，特に筆者らが行っているキヌタ骨による連鎖再建法incus transpositionについて述べた。

　The functional results of one hundred and ten ears of ossicular reconstruction with intact stapes in tympanoplasty are presented. When the malleus was present, free graft of incus was interposed between stapes and malleus in fifty-three ears. The body of the incus was placed on the stapes head and the short process or the shortened long process of the incus was put in to contact the undersurface of the body, neck or handle of the malleus in thirty-five ears.

　This compratively loose connection resulted in good function in the early postoperative stage. But gradual decline in hearing due to discontinuity or fixation was observed. A new method for maintaining of the early functional results must be devised.
