 The sinus pus above the epithelium, the ciliated epithelium, and the connective tissue under the epithelium of chronic sinusitis were obtained operatively and the histochemistry of mucopolysaccharides was investigated. Sinus pus consisted mainly of mucoprotein including cellular and bacterial components. Prozyme and lysozyme were instilled respectively into the maxillar sinuses to decompose mucoprotein and bacterial components. The effectiveness of prozyme judged by contrast radiography was found in 58% of the possible total score, while that of lysozyme was in 11%. Increase of chondroitin sulfate A and C was generally observed in the connective tissue of chronic sinusitis. Testicular hyaluronidase was therefore instilled into the maxillar sinuses. The effectiveness was in 44%. There was no cured case by the direct enzymic therapy, which implied that the ciliated epithelium had a key for conservative therapy of chronic sinusitis.

 The histochemical study of the ciliated epithelium showed that the goblet cell secreted chondroitin sulfate and sialic acid. It seemed to be the most important problem for conservative therapy to control the increase of goblet cell.
