副咽頭腔に発育した巨大な唾液腺混合腫瘍の一症例

A LARGE MIXED TUMOR OCCURING THE RIGHT PARAPHARYNGEAL SPACE, A CASE REPORT

　I．はじめに

　唾液腺に由来する混合腫瘍は唾液腺腫瘍のなかでも高い発生頻度を示す。唾液腺のなかでも発生頻度の高いのは耳下腺で，つぎに顎下腺，その他の小唾液腺の順となつている。混合腫瘍は唾液腺以外の腺組織からも発生することが知られているが，多くは顔面口腔領域に発生する。すなわち鼻腔，副鼻腔，上咽頭，硬口蓋，軟口蓋，咽頭後壁，咽頭側壁，口蓋扁桃，頬部粘膜，頸部などに発生したという報告がある。なかでも小唾液腺の多い口蓋に発生する頻度は高く，学会発表も含めるとかなりの報告がみられる。

　これらの混合腫瘍は良性腫瘍に属するものであるが，しばしば悪性化する。また悪性化しないときでも完全摘出が困難であつた場合には残存腫瘍から再発することが多く，放射線にも感受性が少ないことから予後はかならずしも楽観を許さない。

　著者らは最近，咽頭から頭蓋底にわたる広い範囲に増殖した巨大な唾液腺混合腫瘍の一症例を経験したので，腫瘍の発生について若干の考察を加え報告し，諸賢のご批判をあおぎたい。

　A man, aged 34, complained of a swelling in the right side of the throat for the period of last 4 years. Physical examination showed a mass in the neck which appeared to be connected with that in the throat.

　The tumor was removed surgically in piecemeals through lateral cervical incision. The tumor was enlarged to the extent that it occupied the entire parapharyngeal space and loosely adherent to the base of the skull.

　Microscopic examination of the tumor after removal and of the biopsy specimen revealed the growth to be a mixed tumor of salivary gland origin.
