耳下腺深葉より生じた副咽頭腔混合腫瘍の1症例

A Case of Parapharyngeal Extension of Parotid Tumor

I．はじめに

　耳下腺腫瘍のうち深葉から発生するものは10％程度である。深葉の腫瘍のうちでも副咽頭腔に進展する例は少数である。臨床症状としては咽頭側壁の腫脹が主なので，扁桃炎，扁桃周囲膿瘍と誤まられることがある。今回われわれは耳下腺深葉より生じ副咽頭腔に進展した混合腫瘍の1症例を経験したので，その臨床経過に文献的考察を加えて報告する。

　A 27-year-old male with a parotid tumor of the parapharyngeal extension was reported. The patient complained of a lump in the throat over a ten month period. There was a mass in the left side of the soft palate displacing the tonsil medially. CT-sialography revealed a mass in the left parapharyngeal space. The mass was removed surgically through transparotid approach.

　The shape of the mass was elliptical with small notch on the top of it which was consistent with stylomandibulartunnel, and the pathological diagnosis was pleomorphic adenoma.
