耳性頭蓋内コレステリン嚢腫の1例—頭蓋内に発育したmastoid cyst

INTRACRANIAL CHOLESTEROL CYST, REPORT OF A CASE

　I．はじめに

　慢性中耳炎より乳突部に炎症が波及して，乳突洞ないし乳突蜂巣などにコレステリン肉芽腫を形成することは良く知られている。コレステリン肉芽腫を伴つて嚢腫が発生し，成長してmastoidcystとなることは，まれではあるが起こりうることで，わが国では野村ら^1），三吉ら^2），林ら^3），加我ら^4）などの報告がある。

　著者らは乳突部のコレステリン肉芽腫を伴つたmastoid cystが乳突洞天蓋を破壊して頭蓋内に入り，大きく成長して，側頭骨鱗部の骨を円形に破壊して，左耳上部の骨膜下に出現した症例を経験したので報告する。

　A 35-year-old male patient complained of swelling of the left supra-auricular region.

　The supra-auricular swelling was confirmed as cyst by palpation and needle aspiration.

　X-ray examination revealed a round perforation of the squamous part of the temporal bone, a space occupying mass above the pyramidal bone, destruction of the antral tegmentum and scaphoid erosion of the pyramidal surface.

　Temporal craniectomy showed a large cyst on the pyramidal surface and the cyst was connected with the cholesterol granuloma in the mastoid cavity.

　It was concluded that the mastoid cyst accompanied by the cholesterol granuloma extended into the intracranial space via tegmen antri and became a large cholesterol cyst.

　Some problems concerning causative factors of the cholesterol granuloma and the cholesterol cyst were briefly discussed.
