骨化性内耳炎の一症例

LABYRINTHITIS OSSIFICANS

　Ⅰ．緒言

　慢性中耳炎の経過中，瘻孔症状の出現により限局性内耳炎の疑われる場合が少なくない。しかしそれが慢性のび慢性内耳炎であつて，かつ前庭症状を欠く場合，診断ははなはだ困難である。一般に化膿性内耳炎は急激な内耳症状を現わし，その後，周囲への炎症拡大がなければ，骨化によつて治癒することが知られている。報告の多くは髄膜炎性の内耳炎で，剖検によつて骨化が証明されたものである^1）。本症例は過去に中耳手術を受けてはいるが，これまで明らかな前庭症状を現わしたこともなく，髄膜炎に罹患したこともない。また難聴は中耳炎によるものと考えられ，かつ一側性のためつよい訴えとはならなかつたが結局中耳の再手術により，はじめて骨化性内耳炎が明らかにされたものである。それゆえ，このような慢性内耳炎は，どのような機序で発生するものであろうか，多大の興味が持たれた。したがつて本症例報告は，内耳の慢性炎症と骨化性変化について考察を加えたものである。

　Latent labyrinthitis was found in the course of surgical removal of cholesteatoma on a patient with chronic otitis media. The present involvement is viewed as being tympanogenic in origin. The interesting feature of this case was the obliteration of the inner ear by ossification. The membranous structures of the labyrinth were completely destroyed and they were replaced by a new bone formation except at the vestibule where it was filled with granulation tissues.

　This phenomenon of the new bone formation in the membranous structures is generally accepted as the terminal stage of repair in supurative labyrinthitis.
