鼓室成形術後に生じた激烈な神経性耳痛の一症例

A SEVERE PAROXYSMAL OTALGIA FOLLOWING TYMPANOPLASTY

　Ⅰ．緒言

　外耳道保存鼓室成形術後，4日目に突然激烈なる耳痛を訴え，以後耳痛発作を繰り返した一症例を報告する。一般に中耳手術後にみられる耳痛は一過性であり，もし疼痛が持続，増強する場合は創傷内の血腫形成か膿瘍形成が考えられる。それも術直後から始まる場合が多い。本症例は炎症所見もなく，創傷の術後経過はむしろ良好であつた。

　A case of paroxysmal otalgia is reported. A woman, aged 27, began to complain of a severe pain in the operated ear four days after the tympanoplasty operation. Periodic attacks of pain had been previously limited to the cutaneous supply of the auriculotemporal nerve. The trigger zone was located in the posterio-superior portion of the external auditory canal close the drum membrane.

　A complete relief of symptoms had been afforded by administration of tegretol (carbamyl dibenzoazepin).
