A 7-year-old boy noticed left total deafness and peripheral facial palsy. CT scan showed a destructive lesion of the inner ear in the temporal bone.

At operation, it was found to have a choles-terol granuloma resulted from otitis media with effusion.

The cholesterol granuloma had destroyed the semicircular canal and a large area of the genicu-late ganglion was exposed. Inflammatory granu-loma had compressed the exposed facial nerve. To our knowledge, the cholesterol granuloma involving the temporal bone destruction has rarely been reported. This study calls attention to this possibility.
