滲出性中耳炎により生じた内耳および顔面神経障害例

A Case of Otitis Media with Effusion involving the Cochlear and Facial Nerve

Ｉ．はじめに

　滲出性中耳炎は日常診療においてしばしぼ遭遇する疾患であり，時に合併症あるいは後遺症が問題となる。とくにコレステリン肉芽腫の形成は治癒を遷延させるばかりでなく，稀であるが側頭骨破壊をきたすことが報告されている。

　今回われわれは滲出性中耳炎により生じたと思われるコレステリン肉芽腫の側頭骨破壊で内耳障害および顔面神経麻痺をきたした1例を経験したので報告する。

A 7-year-old boy noticed left total deafness and peripheral facial palsy. CT scan showed a destructive lesion of the inner ear in the temporal bone.

At operation, it was found to have a choles-terol granuloma resulted from otitis media with effusion.

The cholesterol granuloma had destroyed the semicircular canal and a large area of the genicu-late ganglion was exposed. Inflammatory granu-loma had compressed the exposed facial nerve. To our knowledge, the cholesterol granuloma involving the temporal bone destruction has rarely been reported. This study calls attention to this possibility.
