内耳道内に限局した動脈瘤の一症例

AN INTRAMEATAL ANEURYSM, A CASE REPORT

　I．はじめに

　椎骨動脈領域の動脈瘤は一般に稀とされており，特に内耳道内の動脈瘤は稀有である。1967年P. Castaigneら^1）は62歳の女性について，世界第一例目の手術成功例として報告している。この報告のなかには1957年にH. Krayenbuhlらが内耳道内動脈瘤の症例報告をしたことが述べられている。そのほかには1968年にW. E. Hitselbergerら^2）3）が引用した1例があるだけである。

　本症例は，内耳道内動脈瘤が神経耳科学上きわめて興味深い症候を示し，成功裡に手術されたものであり，わが国における最初の報告例である。

A case of intrameatal aneurysm in a female patient, aged 35, which was successfully operated upon is reported. The chief complaints of the patient were tinnitus, hearing loss and facial palsy on the left side. At the operation the aneurysm was found located at the portion where the labyrinthine artery branches off from the inferior cerebellar artery. The growth was removed; and angiography thereafter showed its complete disappearance. The patient lost her hearing completely on the operated side, but the facial palsy made a complete recovery.
