交通外傷による発声障害の4症例

VOICE DISTURBANCES SECONDORY TO TRAFFIC ACCIDENT: REPORT OF 4 CASES

　Ⅰ．緒言

　近年交通量の激増と共に，交通事故の件数も増加の一途を辿つていることは，多くの統計の一致して示すところである。

　交通事故に際して発生する交通外傷について考えてみると，被服におおわれていない頭部，顔面，さらには頸部の外傷はかなり高率に発生し，これらの部分が機能的重要性をもつているだけに，きわめて重篤な結果をもたらすことが多い。そして，このような"首から上"の外傷は，耳鼻咽喉科領域に密接な関係をもつており，多くの問題を提供するものである。

　Four cases of voice disturbance of various natures secondary to traffic accidents are presented with a statistical review of posttraumatic dysphonia in the authors' clinic.

　The causes of dysphonia are; uniateral associated laryngeal palsy due to fracture of the base of the skull, unilateral recurrent nerve palsy due to open neck injury, fracture of the thyroid cartilage with dislocation of the cricoarytenoid joint due to closed injury to the larynx and acute endocrine imbalance resulting in masculinization of voice in a young female, respectively.

　Discussions are made from the standpoint of clinical phoniatrics.
