心房中隔欠損および不完全兎唇を伴つた先天性後鼻孔閉鎖症の1治験例—附：本邦文献例の集計的観察

Atresia Nasi Posterior Congenita Dextra Accompanied by Congenital Heart Disease and Imcomplete Cleft Lip: Report of a Recovered Case

　Ⅰ．緒言

　後鼻孔またはその附近に閉鎖のある場合は後鼻孔閉鎖症といわれ，日常外来ではまれな疾患である。本症はOttoにより1851年に報告されたものが最初とされているが，わが国でも1918年の柳沢の報告以来20数例が報告され，すでに成書にも詳細な記載がある。最近は特に新生児における呼吸と栄養補給の問題および手術的治療法に関する問題が注目される傾向にある。われわれは本邦文献上でもことにまれな心臓奇型および不完全兎唇を伴う先天性骨性後鼻孔閉鎖症の一例に遭遇し，経口蓋法により手術的に治癒させることができたので報告し，あわせて本邦文献例につき検討を加える。

　We succeed in establishing a patient posterior choana of right naris, in a girl aged 13, in whom a congenital occulsion was found due to osseous malformation.

　It must be mentioned that our patient had congenital heart disease (atrial septal defect) and imcomplete cleft lip.

　The patient who had transpalative operation had been completely relieved of discharge and nasal stenosis.

　In our opinion, correction of congenital choanal atresia is best done by a transpalative procedure combined with a large posterior resection of the septum and careful plastic repair of the choanal mucosa.
