先天性両側性後鼻孔閉鎖症の2例

Two Cases of Bilateral Congenital Choanal Atresia

　I．緒言

　先天性後鼻孔閉鎖症は比較的稀な疾患で，1755年Roederer^1）により初めて記載され，続いて1830年Otto^2）により詳細に記述され，1851年にはEmmert^3）によって最初の手術が施行された。本邦では，1923年木村^4）の報告が最初で，以来1980年までに57例の報告があるが，特に新生児期に診断された症例は稀である。今回我々は，新生児期に診断できた2例を経験したので，文献的考察を加え報告する。

　A 5-day-old girl with cleft lip was found having bilateral choanal atresia. At 19 months of age, the transnasal correction was performed, and a rubber tube was inserted in each choana for 4 weeks post-operatively. Four months later bilatiral choanae were obliterated, and transpalatal operation was done and polyethylene tubes (18Fr) were inserted for 3 months postoperatively.

　The second case was a 24-hour-old female infant with first and second branchial arch abnormalities who was diagnosed as having bilateral choanal atresia. At 15 months of age, the transpalatal correction was performed and polyethylene tubes (18 Fr) were inserted for 2 months postoperatively.

　Both cases had no apparent complications after surgery.
