潜在性血友病類似疾患（Plasma thromboplastin antecedent欠乏症）によるアデノイド切除異常後出血の1例

POST-ADENOIDECTOMY HEMORRHAGE DUE TO DEFICIENCY OF PLASMA THROMBOPLASTIN ANTECEDENT

　Ⅰ．緒言

　アデノイド切除，扁桃摘出術は日常臨床上最も頻繁に行われる手術であり，この合併症は我々が最も危惧するものである。手術後出血もその一つであるが，幸いなことに多くの場合術前検査，止血剤の投与などで予防出来るし，後出血が起つても大事に到らず止血し得る。しかし一方種々の注意を払い適切な処置をとつたにも拘わらず不幸な転帰をとるものもないわけではない。私共は術前の検査で出血傾向が強いことを発見出来ずにアデノイド切除，扁桃摘出術を施行し，アデノイド切除創からの大量の出血により一時はシヨック状態に陥り，辛くも一命をとりとめたがその後30日間にわたつて後出血を繰り返えした症例を経験したので報告する。なお血液凝固因子の精査の結果Plasma thromboplastin antecedent（PTA，第I因子）欠乏によるものであることが判明した。

　With a normal preoperative blood examination of red and white cell counts, and, coagulation and bleeding time, adenotonsillectomy was performed on a boy, aged 7. Following the operation postoperative bleeding occurred in large amount from the adenoid area and the patient went into a shock. the shock was soon recovered but a small amount of hemorrhage continued from the adenoid area for the period of 30 days thereafter. The cause of the hemorrhage was due to a deficiency of plasma thromboplastin antecedent.
