扁桃摘出ならびにアデノイド切除と自家中毒症

AUTOINTOXICATION AND SHOCK FOLLOWING ADENOTONSILLECTOMY

　I．はしがき

　扁桃摘出ならびにアデノイド切除の技術は比較的簡単とはいえ，後出血，失血性貧血，脱水症状など術後にわれわれが特に心を傷めることの多い手術である。事故が多く，なお最近は医事紛争にまで発展し易いもので，術前の耳血検査はもちろん，体質，既往症などの詳細な問診，現在の身体状況の十分な観察が要求されるものである。ところがとかく完を尽さない憾があり，私らが以下に述べる症例も多少なりともその例に漏れないものであろう。今まであまり気にしなかつた術後の自家中毒症を引続き3例も経験し辛酸を嘗めさせられたので，あえて報告し，諸賢の注意を喚起したいと思う。

　The authors call attention to the state of shock which at times may occur following the operation of adenotonsillectomy causing vomiting with gastrointestinal hemorrhage, cerebral symptoms and dyspnea that may lead the patient to death. This condition, a Reilly syndrome, which the authors state is avoidable, should be given an early differentiation from the simple postoperative vomiting of blood and mucous.
