急性アデノイド炎について

ACUTE ADENOIDITIS

　I．はじめに

　口蓋扁桃における陰窩性扁桃炎は陰窩に一致して灰白色の膿栓を生じ，さらに癒合して扁桃全体が厚い偽膜様物質で被われることもあり，臨床症状としては高熱，強い嚥下痛，全身倦怠感などがあり，かなり苦痛の強い疾患で，頻度も高く，一般臨床医によく知られている。しかしアデノイドにも，これと同様の変化が起こることは知られておらず，日常臨床においてはほとんど考慮されていないのが現状であろう。成書をみても口蓋扁桃における陰窩性扁桃炎に相当するアデノイドの病変の記載はほとんどなく，特に他の扁桃に著変なくアデノイドのみに強い変化の来ることについては触れられていないようである。

　私は咽頭扁桃にも口蓋扁桃の陰窩性扁桃炎に相当する変化は稀ならず存在することを経験したのでその概略について報告するとともに，耳鼻咽喉科，小児科の臨床にあつては原因不明と思われる高熱には本疾患の存在を考慮すべきであることを強調したい。

　The author reports that during the period of recent 18 months, 46 cases of acute adenoiditis which may be considered as the equivalent of lacunar tonsillitis has been observed. This disease is characterized by occurrance among children aged from 1 to 9, with a sudden rise of temperature 38 to 40 degrees lasting for the period of 2 to 5 days with severe bodily symptoms. The victim suffers stuffy nose with severe impairment of nasal breathing which becomes predominently more troublesome than the rise of temperature.

　The diagnosis is confirmed by inspection of the postnasal adenoid tissues by means of the postnasal mirror.

　The auther emphasizes that the present disease should be considered whenever high temperature continues in infancy without other accountable cause.
