筋注用Trostin Mの耳鼻咽喉科領域における使用経験

SOME EXPERIENCE IN THE USE OF INTRAMUSCULAR TROSTIN M IN THE FIELD OF OTOLARYNGOLOGY

　筋注用止血剤Trostin Mをわが科領域の手術65例に試用して次の結果を得た。

　①扁剔アデノイド切除を主体とする外来手術患者40例にlAmpule使用して，30分後平均出血時間は27秒短縮し，凝固時間は，1分秒11短縮した。

　②同症例群において，出血時間，凝固時間変動と共に術中，術後出血も参考として，著効7例（17．5％），有効14例（35．0％），不確実6例（15．0％），無効13例（32．5％）と判定した。

　③凝固時間の時間的推移を25例について調べ，注射前7分55秒から，注射後30分で7分12秒，60分で7分8秒，120分で7分31秒となつた。

　すなわち，凝固時間を示標としたとき，止血効果は，30〜60分で最大効果を表わすから，術前30分に使用するのが良いと思われる。

　また，その効果は，2時間余にて失われると考えられるから，2時間以上を要する手術では，適宜追加することが望ましい。

　③子供では1Ampule，成人では2Ampule使用で，一応止血効果が認められる。

　④副作用は，認められなかつた。

　Trostin M a hemostotic agent used intramuscularly was tested in operative cases at the Goto Shuji Otolaryngological Clinic.

　Hemostatic activity of this drug measured by coagulation time appeared to be at the peak 30 to 60 minutes after its administration; in operative cases the agent should be administered about 30 minutes before operation. The effect is lost in 2 hours. For operations lasting longer than 2 hours the medication should be repeated. The dosage is 1 ampule for children and 2 for adults; no side-effect was seen.
