鼓室成形術Ⅳ型に対する私共の考えと術後成績

　Ⅰ．緒言

　鼓室成形術が行われるようになつてすでに10年を経過し，その間に種々の研究，改良がなされてきたが未解決の問題も多い。鼓室成形術のうちWullsteinのⅠ，Ⅱ型は成績が良く，Ⅲ，Ⅳ型特にⅣ型は良くないというのが一般的な傾向のようである。しかし，実際問題として私共が取扱わざるを得ない，耳漏を有する慢性中耳炎においては，その治癒のためには耳小骨の除去も余儀なくされることが多く，よくてⅢ型，時にはアブミ骨脚も破壊されてⅣ型にせざるを得ぬ場合も多い。

　今回私共はⅣ型手術の場合，正円窓の遮音に際し全層皮弁を用い，卵円窓部を含む残余術創にはチールシュ弁を接着さす事により，予想外に良結果を得ることができたのでここに発表し御批判を願うしだいである。

In performing type Ⅳ tympanoplasty the authors employed a full thickness skin graft taken from the postauricular area to cover the epitympanic region and Thiersch graft cover-ing the remainder of the middle ear cavity including the oval window. In this way the hearing in 4 cases out of the 7 operated was improved to reach the level within 30db. The fact that type Ⅳ tympanoplasty may attain a better hearing than what had been theore-tically maintained by Bekesy for improvement is discussed with a review of literature.
