耳鼻咽喉科領域フローセン全身麻酔におけるアドレナリン使用の問題

THE PROBLEM ON THE USE OF ADRENALIN IN THE FIELD OF OTOLARYNGOLOGY DURING FLUO-THANE ANESTHESIA.

　はじめに

　フローセン全身麻酔は耳鼻咽喉科領域手術の特殊性を満足させる数多くの利点がある。一方耳鼻咽喉科の手術では，出血防止または止血の目的でアドレナリンを局所的に使用することがきわめて多い。しかるにフローセン麻酔時にアドレナリンを併用することの可否については問題があり，禁忌と考えている人も少なくない。

　われわれは術者として，また麻酔医として全身麻酔による耳鼻咽喉の手術を数多く経験してきたが，その際この問題をどう取り扱ったらよいか，すなわちわれわれが日常使用している程度のアドレナリンが果して循環系に対して無視できない影響を有するかどうかに関して検討する必要を認め，以下述べるような観察を試みた。ここにその概要を報告し御批判を仰ぐ次第である。

1. Adrenalin in 1:250 000 dilution was ad-ministered locally in dosage of 2-40cc (8-160μg) in 203 cases of operations in the field of Oto-laryngology under fluothane anesthesia. A possible effect of the use of adrenalin in this way on the heart action was investigated by means of electrocardiography. The results were nagative except one case in which the development of irregular pulse was thought to be possibly due to adrenalin.

2. From the results of this study it was concluded that there was no adverse effects from the use of adrenalin in fluothane ane-sthesia in the amount which it is usually employed in the daily practice, contrary to the precautions advocated by some anesthe-tists.
