腹腔内に広汎な転移をきたした上顎洞細網肉腫の1症例

　I．緒言

　細網肉腫は比較的近年になつて研究されだしたもので，1924年Komocki, Foot，1925年Gooumaghtigh等による病理組織学的研究に始まり，我が国では1938年緒方（知）により細網肉腫と命名されたものである。その後多くの学者によつて研究され，今日では広くこの名称が用いられている。耳鼻咽喉科領域においてはその性格から本腫瘍が多く発生することは一般に認められているところである。しかしながら上顎洞に発生することは極めて稀とされ，我が国においては昭和16年大沢の報告を始めとして現在までに十数例を認めるにすぎない。

　私共は最近右上顎洞に原発し，その後腹腔臓器に広汎な転移をきたしたと思われる一剖検例を経験したのでここに報告する次第である。

　Reticulosarcoma of the maxillary sinus is rather rare in occurrence. The patient died.

　A man, aged 33, complained of right-sided profuse nasal discharge since 2 months before and pain in the tooth and the cheek on the same side for the duration of 1 month.

　Examination showed the sarcoma to have been originated from the medial wall of the maxillary sinus that caused bony destruction of the oribito-antral wall with mvation of the growth into the orbital fossa. The patient was operated on by partial resection of the maxillary bone and the postoperative progress appeared to be normal.

　On 53 rd postoperative day the patient suddenly developed symptoms of acute peritonitis and died on the 70th day.

　In the post mortem examination metastasis of the sarcoma was found widely distributed in the abdominal organs.

　Thirteen cases of maxillary reticulosarcomas are reported in the literature of this country so far. These cases are discussed.
